I am aware that my child is taking part in this football training program/activity. I understand that a sport such as soccer carries an inherent risk of injury, and that occasionally accidents, falls, and other incidents may result in harm to participants. I indemnify Southern Sydney Soccer from any liability for damages, claims, or losses arising in connection with any injury or harm sustained by my child during participation in the program. I confirm that my child is healthy and physically capable of participating in all activities. I consent to disclosing any relevant medical or psychological conditions to the coaching staff where necessary to support my child’s safety and well-being. I authorise staff members to provide first aid or seek medical assistance for my child if it is deemed necessary in the event of illness or injury. I give permission for Southern Sydney Soccer to capture and use photographs and video footage of my child for marketing and promotional purposes. I understand that all images and recordings will be used in a professional and ethical manner solely for promotional activities. I also agree to receive promotional messages, updates, and marketing communications from Southern Sydney Soccer. I understand that I may withdraw this consent at any time. * Required
